Three structured sessions to help you reflect on your current practice in the area of medical documentation and identify some effective or efficient changes that you would like to start doing in your clinical day or with your team.

Key Learning Objectives & Session Details

Session One

Thursday, September 19th 

5 pm to 6:30 pm PT / 8 pm to 9:30 pm ET

(Live online or view recording)

  • Outline of the mandatory PPEP/PPIP requirements for Canadian Doctors
  • Review the standards of practice in medical documentation in Canada
  • Understand how to conduct a chart audit
  • Review charting efficiency practices that may help you in your clinical day

Session Two

Monday, September 23rd  

5 pm to 7 pm PT / 8 pm to 10 pm ET

(Live online or on your own)

  • Dedicated work time to conduct an audit on your own patient charts alongside peers. We will work quietly together in this session, so that at the end of two hours your audit is completed.
  • Opportunity to ask questions in the chat as you complete your audit.

Session Three

Thursday, September 26th 

5 pm to 6:30 pm PT / 8 pm to 9:30 pm ET

(Required live online)

  • Complete your self-reflection of the audit and practice review
  • Develop an action plan of 2-3 tasks for you or your team to work on
  • Discuss great ideas for change with colleagues in this facilitated session

A structured series of workshops to help you evaluate your practice and review your medical documentation in respect to the Canadian standards for medical documentation.

Have dedicated time to perform an audit of your medical documentation, opportunity for self-reflection,  discussion with peers for information sharing and idea generation and then development of a practice changing action plan.

You can claim Continuing Education Credits for this Quality Improvement Activity and use this activity as part of your PPEP/PPIP requirements.

Can’t make a session?

These sessions are meant to be flexible, and you may still be eligible to complete this series.

The first session must be watched live or on your own time before attending the required third. The recording will be made available to all participants following the live session.

The second session will act as a working session, allowing physicians to ask questions during dedicated work time. This session is optional, as your chart audit can be completed independently if you’d prefer.

The third session is mandatory and must be attended live online for facilitated discussion with peers and attendees are expected to have completed their chart audit and self-reflection.

Meet your host, Dr. Sarah Smith

Dr. Sarah Smith is a rural Family Physician by training and also the Charting Coach for Physicians, Residents and Clinicians since 2018. Dr. Smith has a passion for reducing burnout and overwhelm resulting from the administrative burden of clinical medicine. Using evidence-based coaching, Sarah helps Physicians create sustainable clinical practice within the clinical environments that they work in.
